Pulling down in an empty file will create a list. Pulling down in a file with no visible lists will add a new item to the first (invisible) list and make it visible. Minor cosmetic tweaks.

Adds the notion of a default file and the ability to automatically jump to that file on startup. Fixes list item focus on adding new items.
