Fixed score label bug showing 155/155

Added ability to require a form to be submitted anonymously or turn off a forms ability to be submitted anonymously. Removed quizzes, no one was utilizing this aspect and it caused a slight delay in launching the app.

Fixed new bug on Signature questions

Point based forms when emailed will look at section scores and change the color of the email banner to red if a section is over that sections max score. Forms total score in the yellow range will also change the email banner color to yellow.
