This app allows seniors and/or caregivers to better communicate with their Care Team and Provider in between visits in order to receive more timely care. Seniors are able to track their daily symptoms, submit care requests/needs as well as integrate Fitbit to provide Care Team read-access to the senior's daily health data such as Heart Rate, Heart Rate Variability, Respiration, SPO2, and Activity (steps and sleep data).
