SOAP Notes helps you document a wilderness patient clearly, from your first look at the scene to the evacuation plan. Write a structured SOAP note (Subjective, Objective, Assessment, Plan), log vital signs over time, and turn it all into a verbal report you can read over the radio.
• Structured SOAP note: reporting team, date, time found, location, age, sex, chief complaint, mechanism of injury or history of illness, exam findings
• One-tap GPS fills in latitude and longitude, with accuracy and elevation
