HealthBridge enables you to easily connect and share data from your home health devices with your health care team.
From blood glucose meters to blood pressure cuffs, HealthBridge makes it easy for you and your care professionals to track your ongoing readings, results, and outcomes.
With the app, your connected Bluetooth medical devices automatically share data to your care team. Within the app, your data is recorded and can be reviewed to make it easy to keep track of your progress. Your data is kept secure and your information private – only you and your authorized medical professionals will have access.
Using HealthBridge is easy.
1. Simply download the app.
2. Enter your basic information. (A sign-up code may have been provided)
3. Connect your device(s).
4. From there, your readings will automatically and securely sync to your health care team.
**Usage of the HealthBridge app requires prior enrollment in a program of care by your healthcare provider. The app currently does not support individual usage without consent and support of a healthcare provider. Always consult with your healthcare provider before making any changes to your care plan.**

Introducing patient reminders
- HealthBridge users are now able to set measurement specific reminders that will display as push notifications on the user’s device
- All HealthBridge supported measurement types are eligible for reminders

- Multiple reminders can be set for each day

Primary Goal support (iOS)
- HealthBridge now utilizes the “primary goal” (formerly default goal) that is set for each measurement type in a program to paint goal lines on each graph. Primary goal will also be differentiated in the list of goals that display on the HealthBridge dashboard.

SMS verification
- Users can now verify during registration using either SMS messaging or email.
Push notifications
- When configured for an Impact program, users will now be able to receive push notifications for configured events.
- When you take a reading with a bluetooth connected device, a local push notification will appear to let you know that the reading has been synced to HealthBridge.
Dashboard updates
