The myGrace Haven app connects you with your wellness plan from Grace Haven Medical. Receive daily meal planning, exercise guidance, lifestyle modification support and motivation.
As a Grace Haven Medical patient, you will receive the specific content of your wellness plan in the app. Track your progress including calories, activities, body metrics, steps, water, and sleep in an interface shared with your provider. Communicate with Grace Haven Medical in real time through personal journals and direct private messages.
This app is integrated with the Apple HealthKit. Choose to share your steps, calories expended and other parameters directly with the app.
Intuitive Design
The app is designed specifically for Grace Haven Medical patients and you can't use the app unless you're a patient already in their care. Use the app on your iPhone or iPad.Grace Haven Medical uses an admin web-based portal where they can tailor the program to your needs, monitor your progress, and communicate with you directly.
