This clinical decision-support app unifies two cardiovascular prevention tools: the 2013 ACC/AHA Pooled Cohort Equations (previously known as the ASCVD Risk Estimator) and the American Heart Association’s PREVENT™ Calculator. Combining traditional ASCVD risk modeling with modern cardiovascular-kidney-metabolic (CKM)-inclusive risk equations, CVD Risk Estimator Plus provides clinicians with a comprehensive, personalized cardiovascular disease (CVD) risk assessment.
CVD Risk Estimator Plus computes risk estimates using:
- 2013 ASCVD Pooled Cohort Equations (PCE), which calculate 10 year and lifetime risk of atherosclerotic cardiovascular disease using age, sex, race, blood pressure, cholesterol profile, diabetes status, and smoking status.
- 2023 PREVENT™ Equations, which estimate 10- and 30-year risk for total CVD, ASCVD, and heart failure (HF), incorporating kidney and metabolic health measures such as BMI and estimated glomerular filtration rate (eGFR), and supporting optional predictors including urine albumin creatinine ratio (UACR), hemoglobin A1c (HbA1c), and social deprivation index (SDI).
Clinicians can toggle between Pooled Cohort and PREVENT™ risk trajectories to support guideline aligned prevention conversations. This clinical decision support app provides:
- Risk category classification (low, borderline, intermediate, high).
- Personalized recommendations guided by statin therapy considerations, lifestyle interventions, and risk enhancing clinical factors.
- Export functionality supporting print and email of results for documentation and patient communication.
- Resources page with related tools and informational resources for both clinicians and patients, as well as links to reference documents.
Great look and easy to use! The supporting content is fantastic for patients and clinicians. I will definitely recommend to others!
A new defect

Thenk you for adding a reset button. But now, when we enter a value (which must be done 5 times) it defaults now to the letter type screen, not the numerical. This requires another keystroke for every number that needs to be entered. This may not sound like much, but since we use it for every patient who gets a lipid panel. this is adding up to a pain. It reminds me too much of my old EHR. Can I somehow revert to the previous version? Thankfully another group has one called the "ASCVD suite" which avoids this wrong screen problem.
Doesn't mention bariatric surgery.

Should include bariatric surgery recommendations for patients that qualify. There are quite a few studies looking at CV risk reduction after surgery. The data should be included in this app. This app appears to ignore a major factor that plays a role (obesity) in about 35% (conservatively) of patients.
Totally inaccurate

This new version has some serious flaws. In addition to being harder to navigate than previous versions, it gives seriously incorrect values for ASCVD risk - last two patients I profiled both came back at 1.1% 10 year risk which is just not possible in the population I am seeing. Last person had a total cholesterol of 334! And still came back as 1.1% risk. Please scrap this version and go back to the old one.
This app has been further updated in accordance with the 2026 ACC/AHA Guideline on the Management of Dyslipidemia and now offers treatment advice relevant to the patient's PREVENT-ASCVD and PREVENT-CVD risk estimates.
