The previous reviewer is spot on. I had to re-document the SAME patient 3 times before I figured out where the glitch was. I love the app and can move through it quickly, so I’m in the habit of clicking in the plan of care. Therefore, I do it without thinking sometimes and have lost a couple entries that way too. I know it automatically saves, but, especially considering this new bug and the fact that I’ve lost a great deal of trust in the app with this new problem, it would be great to have a save button that is only for saving while working in a patient’s visit. Currently, when I save, it takes me back to my whole daily schedule. I didn’t even know about the wounds not carrying over. That’s not a good thing at all. PLEASE ADDRESS THIS! You have a great documentation system but these are very frustrating issues that take a lot more time now. The only way I’ve found to use the app is to do everything except my interventions for the visit (poc) in the app, then go back to the web based version to finish that part. Furthermore, the web version frequently doesn’t work for me. It’s a big issue! Thank you.

All of the reviews about this app still stand true today, 4/8/25. Charted an entire assessment in the app, saved, went to review the meds and all of the assessment was deleted. You also can’t do many of the functions that you can in the browser version; things that are essential like discontinuing a medication or reviewing previous values from past assessment in your current assessment. Several small things, that seem to be intuitive, have completely been missed with the developers, like posting patient’s contacts number under their names. You have to instead click edit to see the number. There are MANY very simple things like this in the app that have been missed. Very clearly not designed or maintained by nurses, and most likely anyone with true healthcare experience. These problems have clearly been an issue for sometime per the negative reviews and their respective dates. I’m willing to bet these issues will never be resolved since there’s been several “bug” fixes with all of the posted updates, yet all the bugs still exist. If you have to use this charting system, just avoid the app altogether and stick with the much less buggy browser version.

I have to restart my phone after one visit. The next visit will not let me save and exit, losing all the work for that visit.

Updating my review 7/18: this app is terrible. Do yall even read our reviews and attempt to fix the problems? This is my 3rd time over the past few months notifying of problems…. The only thing I’ve seen fixed is the ability to document on care plans from the app. The web version isn’t much better either as far as problems go. GET IT TOGETHER. The new update yesterday made us lose the ability to chart on care plans and put orders in- the app forces closed and we lose everything we charted. Also when we chart on wounds from the app, it does NOT pull to the chart. Completed visit looks as if we never charted a wound. These are the bug fixes we need! Now we cannot chart at all on the app… it will not save or complete.. only discard changes works. We are losing full visits. I’m changing my star to 1. Every time this app updates to fix bugs it adds more and we lose more abilities. Get it together!!!
