The program has been good so far and I have nothing but positive things to say about it. I was forced to use this program because it’s the only way insurance will cover it. But overall, it’s been very easy and convenient. There’s nothing very impressive about the app. It is very basic. One issue I have in particular is in the messaging. When I message back-and-forth with my physician, it creates multiple duplicates of my message, and sometimes they’re out of order. Considering this is dealing with peoples medical records, I would hope they would have a more stable chat system.

The app does not notify me when I have a message. I have to remember to open the app and check, missed a couple days of messages and instructionsApp other wise is friendly but the program seems to mimic the previous reviews. No one told us our insurance weightloss program even changed so by the time I was forced to go thru this program I had already been frustrated for months trying to get help on my own. took over 2 weeks to even get a team assigned to me and will take another 3 weeks to even get any kind of appointment. I work in retail sales and my hours are all over and the staff don’t have flexible virtual appt times for different schedules. I get a better response from my doctor when there’s a holiday weekend. This feels like a blatant money grab. Delay everything so long so my company pays them

Wow! I’m shocked by these bad reviews. My insurance also required me to join to get any of the WL shots covered anymore (even though they had been covered by my Dr’s prescription). But after I got set up, I got an appointment pretty quick with the Dr assigned to me. It took about 45 minutes total for the initial apt. My dietician apt is this Friday, which didn’t take long either. My dr has already put in a prescription for me. True, you take pics of your meals, but I also enter what I’m eating in MFP (I do wish this ap had similar). I just screenshot that & upload. There’s a place for notes to put the date. So far 5 stars. ⭐️⭐️⭐️⭐️⭐️

I have gotten wonderful support from my team and have had no issues with the app. I was surprised to see the current Meh rating. I’ve worked with Form for 8 months now and plan to continue to do so as I get closer to my health objectives. I’m down 40 lbs, my broken foot has finally healed, plantar fasciitis is gone, my labs look great. I couldn’t be happier, for me this company is a great fit.
